Abstract: 

Introduction: Urinomas are rare and uncommon lesions that occur in the ureter or around the kidney. The aim of this study was to report a case of a rare URETERAL urinoma. Case Presentation: The patient was a 69-year-old woman who referred to a medical center complaining of left-side pain. The results of the patient's tests showed low creatinine and higher than normal urea. According to the abdominal and pelvic ultrasound findings, URETERAL urinoma was diagnosed. The patient was discharged from the hospital after taking the prescribed medicines on November 26, 2022. Conclusion: The findings of this study can help the treatment staff to consider the possibility of diagnosing urinoma in patients who present with symptoms of fatigue and flank pain. Therefore, being aware of the symptoms of this disease will help diagnose these patients faster and prevent the disease from worsening.

Abstract: 

PROPOSE: MANY SMALLER FLEXIBLE OR RIGID INSTRUMENTS MAY PASSED WITHOUT PRIOR DILATION DEPENDING ON THE SIZE OF URETER. ALTHOUGH MOST ORIFICES ARE APPROXIMATELY 3MM IN SIZE, SOME ARE SMALLER. BECAUSE THE SIZE IS NOT PREDICTABLE, MOST AUTHORITIES ROUTINELY DILATE BEFORE A URETEROSCOPIC PROCEDURE. METHODS & MATERIALS: CYSTOSCOPY (NO.22FR. STORZ OR NO.21FR. WOLF SHEAT) BEFORE URETEROSCOPY DONE FOR ALL PATIENTS CANDIDATES FOR TUL OR OTHER URETEROSCOPIC PROCEDURES AND SUCCESSIVELY LARGER URETERAL CATHETERS NO.4, 6, 8, 10 PASSED TO URETER. IN CHILDREN CYSTOSCOPE 12FR. WERE USED. RESULTS: DURING A PERIOD OF 28 MONTHS 578 PATIENTS UNDERGONE URETERAL DILATION BEFORE TUL OR OTHER URETEROSCOPIC PROCEDURES.30 OF THEM WERE 2-15 YEARS OLD AGE. URETEROSCOPY AFTER URETERAL DILATION IS EASIER AND MORE SAFE SPECIALLY IN CHILDREN. IT SAVES ABOUT 200,000 RLS (25 US$) PER PATIENT. FALSE PASSAGE OCCURRED IN 48 CASES AND STONE MIGRATION IN 18 CASES. DISCUSSION & CONCLUSION: DILATION EXPANDS THE SAFETY MARGIN AS THE URETEROSCOPES ARE PASSED, AND IT ALLOWS LARGER FRAGMENTS OF CALCULI TO BE REMOVED. SUCCESSIVELY LARGER URETERAL CATHETERS IS A SAFE AND SIMPLE AND INEXPENSIVE METHOD FOR URETERAL DILATION.

Abstract: 

Purpose: Radical open nephroureterectomy (ONU) with bladder cuff excision (BCE) is the traditional gold standard approach for management of high-risk non-metastatic upper tract urothelial cancer. ONU involves two separate procedures; the nephrectomy and distal ureterectomy, with each of these parts being able to be performed with an open or minimally-invasive approach. Multiple approaches have been described for the resection of the distal ureter and bladder cuff after mobilization of the kidney and upper ureter. Materials and Methods: A Medline search of the literature including relevant articles up to March, 2020 was performed. Search terms included “ nephroureterectomy” , “ upper tract urothelial carcinoma” , “ upper urinary tract carcinoma OR UTUC” , “ open OR conventional OR ONU OR conventional” , “ robotic-assisted nephroureterectomy OR RANU” , “ laparoscop* OR LNU OR LRNU” and “ minimally-invasive nephroureterectomy” . Original articles, case series and review articles were included. Results: There are no randomised studies. Various techniques have been described to manage the distal ureter during nephroureterectomy. This review provides an overview of these techniques. The perioperative and oncological outcomes following open versus endoscopic techniques and minimally invasive techniques have been described. Although endoscopic approaches have more favourable perioperative outcomes, this comes at the expense of increased risk of tumour spillage and recurrence compared to the traditional open approaches. Minimally-invasive techniques (laparoscopic and robotic-assisted NU) largely have superior perioperative outcomes versus their open NU counterparts, with comparable oncological outcomes. Conclusion: Current non-randomised evidence is open to selection bias and is insufficient to support or refute endoscopic management of the distal ureter as an alternative to open bladder cuff excision. The optimal approach to nephroureterectomy and management of the distal ureter continues to remain a surgical dilemma.

Abstract: 

Introduction: During recent years, considerable efforts have been expended into the management of urinary stone. Here, we present our experience on ureteric stone removal without any lithotripsy interventions. Combination direct vision with basket en-trapping provided a new dimension to our ureteroscope experience. Materials and methods: Here, we reviewed the medical data of our adult patients with ≤ 10 mm stone size, who received primary stone extraction under direct ureteroscopic vision without lithotripsy during a 2-year period. During the six months of follow-up every patient was seen frequently. Results: The study included 69 patients from both sexes with ages ranging from 18 to 68 years. We obtained 92. 7% success rate. The average length of operative procedures was 25. 3 ± 10. 4 min with a 14. 4 % complication rate. Conclusion: URETERAL stone extraction requires considerable caution and may be associated with some complications. Stone extraction under direct ureteroscope guidance facilitates this procedure, especially in the distal stones. It seems combination direct live imaging with basket en-trapping may be helpful in these precise cases.
